This is a detailed appointment with Dr. Cindy Jakubiec to go through a comprehensive health history and your most recent blood work (if available, please bring to your appointment). A complete discussion of the program will take place. During this time your questions will be answered for you to begin the program as soon as you are ready. You will receive one Complete Patient Guidebook and one Shape ReClaimed Nanomolecular Dietary Supplement. An initial urine analysis will be performed, the results will also be discussed during this appointment.

Dr. Cindy Jakubiec is a chiropractic physician who’s purpose is to help patients restore and revitalize their health through chiropractic, holistic health, nutrition and functional medicine with the ultimate goal of embracing a healthier lifestyle and regaining their health. Dr. Cindy Jakubiec is a licensed Chiropractor and has been in practice since 2001.

Dr. Cindy Jakubiec is also certified in Applied Kinesiology, Bio-Energetic Synchronization Technique, Acupuncture, Webster Technique and Shape ReClaimed.

Dr. Cindy Jakubiec was introduced to chiropractic at an early age. At four years old she had a bad accident at home and by nine years old she had horrible migraines and back pain that kept her home from school. Because of her mother’s intuition and “knowing she wasn’t ‘sick’”, she began chiropractic care. Even at nine years old she understood the profound impact of chiropractic and (as she says) how “it saved my life!”
